Never terminate a probationer. Instead, just write "No able to confirm". e.g. Despite of the previous communication of your non-performance from the company, and the ample opportunity for you to prove yourself given by the company, you still failed to prove that you are a good fit for the company. Therefore, we regret to inform you that you will not be confirmed of your position, and the company has decided not to extend your probation period.
