Go to nhrc.com.my - its a sub-unit of HRDF and they provide samples and even free consulting. If you are an MEF member (most manufacturing companies are) they have loads of information that you can refer, policies, practices etc.

Yup. LOA or Employment Contract is a Contract (duh...), so it is governed by Contracts Act 1950. If the employer is a company, then it is also governed by Companies Act 2016. That means it must be signed by an Authorized Person on behalf of the employer.
