Dear HR experts, this is a performance improvement table where there is no column for the employee to put her comments, is this correct? HOD says its not necessary, employee is seeking our human capitals assistance and we are not sure?

It would be good to have a rating as well. So that it could be a clear indication of the performance achievement. It has to be mutually agreed both employee and employer so I think its good to include employee remarks to know his feedback. And the assessment is done both ways not just from the employer point of views. My 2 cents.
