Do they need form for PERKESO Covid-19 Test?

For the perkeso test covid-19,   it is have any form need to fill up?  Any panel clinic? Is free right? Or we should go for testing and claim from perkeso?

only for foreigners from construction need to do swab test Covid-19. SOCSO already announced there will be no testing for antibody for locals, as planned before.
