What's the purpose of health declaration?

Are we going to hold the person responsible if he gave a false declaration?

Asking them to fill the declaration form is for our record in case anyone of the employee got infected and that's the document we can refer

At least we take precaution b4 they enter to our premises, if they hv bn found that they hv sympthom by checking their temperature or they hv travel on the suspected area, we can prohibited them from entering. as for own staff the format might hv to change a bit based on the requirement
