Do we need to issue a letter if staff did not pass probation?

may I know do you issue a letter when staff does not pass probation? or just officiall email + probation appraisal form which indicates she does not pass will do?

Notification in writing i.e. by letter to the staff informing of the extension, etc., and the reason why. Please do not end the probation and just tell the staff that he did not pass probation and end his employment. altho the contract states that, there is due process to follow i.e. company have to give chance to the employee to correct/improve his weaknesses
