Sick leave must be at least 4 days before you can claim. If the accident is claimable under SOCSO, fill up the Borang 34, attach the ORIGINAL sick leave, photocopy of attendance record/punch card (for the month the accident happened), preceding 6 months payslips, photocopy of employee's bank statement showing his bank account no. & photocopy of employee's I.C. All documents must be in A4 size. The last page of Borang 34 needs to be signed by the employee too. Make sure SOCSO acknowledged receipt of the documents submitted as the sick leave is ORIGINAL copy in case they misplaced your submission. In other words, better do a cover letter listing all the documents which you hv submitted & get the SOCSO office to acknowledge receipt.
