What is the normal notice period for resignation?

Hi all. Would like to ask if my staff resigns with one month's notice with letter dated 7 Nov. He has 5 days of leave balance. So when will be his last day? How to mention this in letter? Can I have a sample letter for my easy understanding? Thank you.

Normal notice period - last day shall be 06 Dec, three way to settle for balance leave: 1. clear during notice period, 2. offset- meaning physical last working day will be 01 Dec, salary pay till 06 Dec. 3. serve as normal notice , work until 06 Dec, but pay the employee with monetary.
