The application in WSP should put in which column?

On 2nd column  Tuntutan Ke (1,2,3,4,5). Do I put 1 for this application? I understand that we only apply once and advise changes if any for succeeding months. So, for May we still need to submit and put 2?

You are right! This is refer to the number of claim by staff. So if Staff Abc claim from April, should put 1, next month will be no, 2. and so on.
