What is the practice at your company on the issuance of show cause letter?

Meaning if an immediate superior report to HR, do the HR Manager sign/issue the show cause or should MD sign it?

Please correct me if i am wrong: if HOD report to HR about staff issue, HR shall arrange to have counselling, and need to investigate on the matter, if staff have guilty , HR cause issue Show cause letter to staff and HR can sign, get the staff to reply within 2-7 working days, depend on the case how serious it is.
