May I know testimonial letter for resigned employee normally is issue by HOD or HR department?

HR will only give a letter confirming that he was an employee for a said duration... How can HR give testimonial letter when HR doesn't know how his working attitude was. But a copy should be sent to HR for file

Issued by HR Department. HOD can also issue to their department employees Letter differs between HR and HOD. HR will certify the duration of services with the organisation
