Dear all, is need to fill up form if employee still want remain on 11% contributions? How to get the form? I try to seach the form17A but it’s not in the website yet.

individually complete? Can we do a group submission?

HR can collect all the completed forms and submit them to the nearest EPF branch.

For employer's contribution, can be done by listing out the names

If I not mistaken, for employee's contribution, should be done individually.

To remain 11%, must complete the form and submit to the nearest EPF branch. You can download the form from KWSP website.
